When staying home is no longer an option, Chris Jensen offers a wide array of services to support residents in our caring community. We help each person to achieve and maintain an optimum level of independence by developing and implementing a personalized care plan. Our professional health care team’s goal is to create home for the people we care for while working closely with their families.
Short Term Rehabilitation: For adults who n need inpatient therapy and/or skilled nursing services following surgery, injury or illness, Chris Jensen provides a place for recovery with the goal of returning home as soon as possible. Physical, occupational and speech therapy services are offered up to seven days a week on an inpatient as well as on an outpatient basis. Our primary objective is to assist each individual in achieving and maintaining his/her highest level of independence and safety.
Willows Memory Care: Our memory care environment and specialized programs provide the right balance of independence and safety for individuals facing Alzheimer’s disease and/or other types of dementia. Our trained staff provides individualized care which reflects the person’s usual daily lifestyle. A variety of activities are designed to support the strengths of those we serve.
Chris Jensen Health and Rehabilitation Center employs an interdisciplinary care giving model whereby each resident receives the dedicated attention of a multi-skilled team which consists of nursing, social service, dietary, activities, and rehabilitative therapy. What really sets our team apart from care giving staff at other care centers is our experience and longevity. Many of our professional caregivers and department leaders have been with Chris Jensen for a decade or more.

Located in a park like setting, Chris Jensen Health & Rehabilitation Center has a long history of commitment to providing services and care to those in need. On this site in the early 1880's, the "poor farm" was built, housing 36 people. Warm beds and hot meals were available to those who worked with the livestock and gardens. Additions were made to the building, now "Cook Home", and in the height of the Depression it housed up to 612 people. Over the years a transition occurred. The facility no longer just provided housing, but nursing care also. A new building, Chris Jensen Nursing Home, was built on the campus as the need for nursing care increased. The "old" Cook Home was demolished in 1983. Chris Jensen Health & Rehabilitation Center has established itself as a leading provider of short term rehabilitation services on its specialty unit, memory care, and long term care. We continue to carry on the mission of St. Louis County by providing services to those in need.

- How do I place a loved one in a nursing home?
- Finding a new home for a family member is difficult whether it be for long or short term care. We believe it is very important for you to tour the nursing home. Stop and ask questions of the staff. At Chris Jensen we have an admissions coordinator who will walk you through the steps of placement. The first step is to give her a call at: 218.269.5233. Cindy will do an assessment of your family member to help us better understand the care needs and help us personalize her/his admission. The Admission Coordinator will discuss with you your insurance plan ~ helping you understand the cost and how payment is determined. Perhaps you don’t have a clue how you’re going to pay the bill, Cindy; the Admissions Coordinator will talk to you about Medicare and the Medical Assistance programs.
- What is the admission process?
- To enter a nursing home in the state of MN you have to be seen by your physician who states you require nursing home place. Call our admissions coordinator and she will share with you the admission paperwork that your physician will need to complete prior to admission. At the day of admission, our team of social workers will meet with you and discuss placement. We want to know as much about the new resident as possible so we can start to develop a plan of care for them.
- What services are provided?
- Chris Jensen is a skilled nursing home providing 24 hour care. We employee over 200 staff who meet the needs of the residents 24 hours a day. We offer a secured memory care unit for people that have memory issues and benefit from a safe environment. Our short-term care unit is designed to work closely with the team of therapist, nurses and social workers to assist them to discharge back to the community safely. For residents who suffer from diseases that make their decision abilities less, we offer a unit that provides safety, security, socialization and a sense of worth.
- How would I spend the day at Chris Jensen?
- Each resident who resides at Chris Jensen has a plan of care developed for them. Each department: dietary, social services, nursing, therapy, activities will work with the resident and family to determine what they want to do to allow them to be as independent as possible during their stay. This plan of care is updated with the resident every three months. An example of a plan of care is: A resident doesn’t like to get up at 7-8 for breakfast. So their plan of care would allow them to have the ‘rise and dine’ breakfast. Meaning when they wake up at the time of their choosing they will be offered a continental breakfast. Activities will determine different likes and dislikes ~ are they religious and want to be invited to the many spiritual groups that we have involved at Chris Jensen. They will determine if they prefer to attend large groups or small group activities. Groups may be: entertainment, music, bingo, cards, cooking, exercise, games, outdoor, outings. Each resident is given a monthly calendar so they can plan their day or week. The therapy department will be involved if the interdisciplinary team feels there is a need. The team makes a request to the physician and if agreed, the therapy department will work with the individual perhaps on balance, walking, dressing, strengthening, home safety or many other areas. The social service department is available to be the liaison between you and other departments to help problem solve issues, concerns. The social worker is also the person who will assist you to return home if that is what the plan is. Nursing is the department that you will work with the most. They will assist you with all your cares and medical needs as determined by your physician, you and your team at Chris Jensen.
- How do I pay for nursing home care?
- Chris Jensen accepts Medicare, Medical Assistance, Private Pay, many approved insurances.
Medicare: To quality for Medicare you need to have spent 3 midnights in the last 30 days. You also need to have a skilled provided. Medicare will pay for room and board, therapy and medications at 100% for the first 20 days. After day 20 there is a daily co-payment. There is NO GUARANTEE that you will receive a full 100 days of coverage.
Medical Assistance: This is based on your finances. This is a program that you apply through the county for. Usually when you are on Medical Assistance you have to turn over your monthly income and are allowed to keep a minimal amount monthly. The program does look at your situation if you are married, short term stay, under-age children at home etc. To determine if you are eligible you need to complete an applicant at St. Louis County.
Insurances: The Admissions Coordinator will work with you to determine if your insurance is approved for our facility.
